PROCEDURES FOR SPORTS CLEARANCE

 PLEASE READ CAREFULLY

Parents, Complete Your Athletics Registration Online!

All students must be registered for EACH SEASON on the Athletics Registration site found on the Wayne Hills High School Page - Athletics

Please read and follow the instructions carefully to be fully registered.  You are only registered once ALL sections are completed.  

IMPORTANT:  The username and password you set up is for all family members.  Please be sure to record your username and password for future registrations.  

Step 1:    Complete the online registration.    STEP 1 REGISTER: FOR FIRST TIME ATHLETES: Create an online account and register HERE Please follow the prompts to create an account. Click on Winter Sports Registration and fill in the requested information   FOR RETURNING ATHLETES: Use your account ID HERE Your registration will auto populate. Click on Winter Sports Registration and select the new season sport. IMPORTANT:  There should be only one account per family.  The username and password you set up will be used for future registrations. You must contact rSchool at 1-800-513-5220 to merge multiple family or student accounts.    Step 2:    Determine what forms must be submitted to the health office/nurse. All forms must have the athlete's name where indicated.   Pre-participation Physical Evaluation Forms:  History Form: is completed and signed by the athlete and parent/guardian and reviewed by the healthcare provider during the physical exam. Special Needs Supplemental History Form: is completed and signed by the athlete and parent/guardian and reviewed by the healthcare provider during the physical exam. Physical Examination Form and Clearance Form: completed by healthcare provider (MD, DO, APN or PA), signed and dated.  The date of exam must be filled in where indicated.  The office stamp must be on the Clearance Form. Health History Update Questionnaire:  This form is required from each student whose physical examination was completed more than 90 days prior to the first day of official practice. It must be signed and dated by the parent/guardian.   First Official Practice Dates Baseball / Softball (Pitchers & Catchers) March 11 All Sports: March 14  NOTE:  Athletic physicals are current for 365 days from the date of exam, not the date the paperwork was signed by the doctor. .     Step 3:  Submit all forms to the nurses in the health office.   Electronic submissions will not be accepted. Students will be permitted to participate when all required forms are submitted to the health office. Parents/guardians will be notified by email when their child has been cleared to participate in athletics.    All forms must be submitted DIRECTLY to the School Nurse  PLEASE SUBMIT FORMS AS EARLY AS POSSIBLE TO PREVENT POTENTIAL DELAYS IN THE STUDENT ATHLETE'S PARTICIPATION.  IMPACT Baseline Test:  All student-athletes are required to take an online concussion baseline test (IMPACT).  The Athletic Trainers will be responsible for scheduling and administering the test.  This test is valid for 2 years.  IMPORTANT REMINDER:  Students who are excluded from PE participation for any reason are not permitted to participate in athletics Google Classroom Codes - Information on specific sports.
