Registration and Medical Clearance Information for High School Athletics
IMPORTANT UPDATE: Athletic registration platform has changed effective immediately. rSchools has been replaced at the state level with a new program called Arbiter. Any athletic registrations that have been completed through rSchools cannot be transferred, therefore all athletics registrations must be submitted through Arbiter. Please follow the links below to create your new account with Arbiter and follow the steps carefully to ensure your student-athlete is cleared for participation.

REQUIRED HEALTH FORMS
Pre-Participation Physical Exam Packet- Submit only the Medical Eligibility Form to Health Office
We kindly request that parents/guardians consider submitting the complete physical exam packet. While only the Medical Eligibility Form is required, submitting the Physical Examination Form and History Form as well helps us gain a more comprehensive understanding of your child’s health. Your consideration is greatly appreciated and supports our ability to provide the best care possible.
Health History Questionnaire - complete and submit if the student athlete’s physical exam was completed more than 90 days prior to the first official practice date.
SUBMIT ALL COMPLETED ORIGINAL FORMS TO THE HEALTH OFFICE
NO ELECTRONIC COPIES WILL BE ACCEPTED
CLEARANCE TO PARTICIPATE IN ATHLETICS
Please note: Middle School Nurses do NOT have authorization to provide athletics clearance.
Once registration for the sport through Arbiter is verified, the Medical Eligibility Form and/or the Health History Update Questionnaire have been reviewed and approved by the School Nurse at the High School AND the School Physician- a clearance letter to participate will be sent to the parent/guardian via email.
