Registration and Sports Clearance Information for High School Athletics


Dear Parent/Guardian:


The NJDOE has changed the Health Forms required for sports clearance. The previous forms will no longer be accepted starting July 1. In an effort to ensure all of our new and returning student athletes are cleared to participate in their sport prior to the first official practice, please read this letter carefully and in its entirety taking note of the new procedures and forms.

  3. PHYSICAL EXAMINATION FORM- to be completed, signed, and dated by the student athlete’s healthcare provider. 


The Physical Examination is valid for 365 days from the date of the exam. Please take note of the following official practice start dates to ensure your student athlete’s physical will be valid or if a new physical examination will be needed.

  1. PREPARTICIPATION PHYSICAL EVALUATION MEDICAL ELIGIBILITY FORM- to be completed, signed, and dated by the student athlete’s healthcare provider.

  2. HEALTH HISTORY UPDATE QUESTIONNAIRE- to be completed, signed, and dated by the student athlete’s parent/guardian if the student athlete’s physical exam was completed more than 90 days prior to the first official practice date.

ImPACT® CONCUSSION BASELINE TESTING

ImPACT® Concussion Baseline Testing is to be completed by ALL student athletes. The Athletic Trainers will schedule and administer the computer - based testing for all new and returning athletes. The ImPACT® testing is valid for two years. 

CLEARANCE TO PARTICIPATE IN ATHLETICS

Once registration for the sport through rSchoolsToday is verified, the Medical Eligibility Form and the Health History Update Questionnaire have been reviewed and approved by the High School Nurse(s) AND the School Physician- a clearance letter to participate will be sent to the parent/guardian via email. 

Your child will NOT be permitted to participate in official practice without clearance.


NOTE:  Athletic physicals are current for 365 days from the date of exam, not the date the paperwork was signed by the doctor. .  

 

Step 3:  Submit all forms to the nurses in the health office.   Electronic submissions will not be accepted.

  • Students will be permitted to participate when all required forms are submitted to the health office.
  • Parents/guardians will be notified by email when their child has been cleared to participate in athletics. 

 

All forms must be submitted DIRECTLY to the School Nurse 

PLEASE SUBMIT FORMS AS EARLY AS POSSIBLE TO PREVENT POTENTIAL DELAYS IN THE STUDENT ATHLETE'S PARTICIPATION. 


IMPACT Baseline Test:  All student-athletes are required to take an online concussion baseline test (IMPACT).  The Athletic Trainers will be responsible for scheduling and administering the test.  This test is valid for 2 years. 

IMPORTANT REMINDER:  Students who are excluded from PE participation for any reason are not permitted to participate in athletics
